The services are listed by charge. These are the cheapest premiums and require no claim forms.

Employee +1 $10.00 per pay period
Employee + Family $14.00 per pay period

Elite Choice 75 - Indemnity Plan -- You go to a licensed dentist and they reimburse you for what you pay. YOU HAVE TO FILE CLAIM FORMS.

Employee $12.00 per pay period
Employee +1 $20.00 per pay period
Employee + Family

$28.00 per pay period

Be advised some dentists are in the plan but not accepting new patients. Be sure and call them even if it says they are not. We have found where this website is sometimes wrong and some dentists do accept even when listed as not.
